Practitioner represents a provider in all three FHIR outputs, but its role differs. In Bulk Medication History it is always the prescriber of a medication and is a standalone resource. In the Patient Everything and Patient Summary bundles it represents any provider associated with a clinical record — and is contained rather than standalone. The Stable ID and Fields sections below cover both, with a column per output.

Stable ID

Neither id scheme deduplicates by the provider’s real-world identity. In the bundles, the same provider appears as a separate contained Practitioner under every resource they are associated with — use the NPI in identifier to correlate them. In Bulk Med History, the same prescriber appears as a separate Practitioner resource for every medication they prescribed within a file.

Fields

In the bundles, a Practitioner is only produced when the provider has at least an NPI or a name; telecom, address, and qualification are not populated — contact details appear on the surrounding PractitionerRole instead. In Bulk Med History, a Practitioner is always produced for the prescriber.
