This resource carries the assessment and plan of treatment narrative in the Patient Everything and Patient Summary bundles. It is not produced for Bulk Medication History.

Stable ID

The id is derived from the patient, the plan date, and the plan narrative text. Because the narrative is part of the id, any edit to the plan text produces a new CarePlan rather than updating the existing one.

The encounter link described below is not part of the id, so the same plan carries the same id in both bundles.

`text.div`	1..1	The plan narrative, HTML-escaped so the source text is rendered as literal text rather than interpreted as markup.
`text.status`	1..1	Always `additional` — the narrative carries information beyond the structured fields, which is the whole point of this resource.
`author`	0..1	Contained PractitionerRole for the provider who recorded the plan. Omitted when no provider is known.
`encounter`	0..1	Patient Summary only. References the Encounter for the visit the plan was recorded at. Never set in the Patient Everything bundle.

CarePlan is a narrative-only resource here. activity, goal, addresses, period, and careTeam are not populated — the entire clinical content is the text.div narrative. Consumers should not expect structured activities or goals.

Patient Everything	All plans of care on the patient’s record, independent of any visit.
Patient Summary	Only plans attached to a visit that was selected for the summary — including plans on follow-up visits. Each is linked to its visit via `encounter`.
